FORMS AND DOCUMENTS FOR THE FUND ADMINISTRATOR

  • Award Check Request Form:
    Download and submit this form to request an Award Check from your Memorial Fund.
  • Automatic Withdrawal Authorization Form:
    Download and submit this form to authorize CHARITYSMITH to withdraw the annual administrative fee of $240 directly from the Wells Fargo Bank Account associated to the Memorial Fund you administer.
  • Expense Reimbursement Preapproval Form:
    Occasionally, you may have expenses requiring reimbursement from the Memorial Fund. These reimbursements must be pre-approved by CHARITYSMITH prior to incurring the expense. Download and submit this form to request a reimbursement pre-approval from the Memorial Fund.
  • Award Receipt Verification Form:
    Download and submit this form to record award recipients from the Fund.
  • Cash Donation Receipt Form:
    Download and complete this form and send to CHARITYSMITH at P.O. Box 10750, Bainbridge Island, WA 98110 with the funds to be deposited. Do not send cash through the mail. Enclose a cashier’s check or personal check for the exact amount of the donation and return it with this form. In the memo portion of the check include the name of the donor. The donor should retain the bottom portion of this page as a receipt.
