Nomination Instructions:

Write an essay in which you nominate yourself or someone you know to receive the Gardner Family Medical Science Scholarship. CHARITYSMITH is particularly interested in those who have demonstrated persistence and perseverance in the process of applying to medical school.

Write the essay in black or blue pen on a single piece of 8.5” x 11” paper. (You may use college ruled paper, blank paper, etc.)

Write the essay in a single sitting of 60 minutes or less. (Please note that while spelling and grammar are certainly important, we are more interested in your experience as a medical school applicant. Please do not waste your own time, or another’s, by having the essay corrected and rewritten.)

In the top right corner of your essay, be sure to clearly provide:
a. Your full name
b. Your permanent address
c. Your current phone number with area code
d. Your permanent phone number with area code (i.e. parent’s number)
e. Your email address

Nominations must be postmarked on or before May 15.

Notification of the recipient of the Gardner Family Medical Science Scholarship will occur by email and telephone on or before June 1.
Please note: Essays that do not concord with these instructions will not qualify for consideration. Please be sure to accurately follow these instructions.
